This retrospective study aimed to evaluate the association between the oxygenation index (OI) and survival outcomes in dogs undergoing mechanical ventilation for primary pulmonary parenchymal disease. The study included 58 client-owned dogs that received positive pressure ventilation due to inadequate oxygenation from pulmonary parenchymal disease. The median duration of positive pressure ventilation was 30 hours, with a range from 10 to 136 hours. The overall survival rate was 32%, with 19 dogs surviving to discharge. Survivors were significantly younger and had higher median PaO₂/FiO₂ ratios at 4 to 12 hours post-initiation of positive pressure ventilation and immediately before weaning compared to non-survivors. A receiver operating characteristic analysis of the PaO₂/FiO₂ ratio immediately before weaning as a predictor of survival had an area under the curve of 0.76, with an optimal cut-off point of 252 mmHg, corresponding to a sensitivity and specificity of 80% and 79%, respectively. The study concluded that the PaO₂/FiO₂ ratio is an early prognostic indicator of successful weaning in dogs undergoing positive pressure ventilation.
