This retrospective study evaluated complications and long-term outcomes of combined tibial plateau leveling osteotomy and tibial tuberosity transposition (TPLO-TTT) in 24 stifles from 22 dogs with concurrent cranial cruciate ligament rupture and grade III-IV medial patellar luxation. Four major complications were documented: three surgical site infections successfully managed with implant removal and antibiotics, and one recurrent grade II medial patellar luxation that remained clinically asymptomatic without requiring additional intervention. Five minor complications were also noted. At long-term follow-up (median 27 months, range 12-67 months), 21 of 22 dogs achieved clinical soundness and 23 of 24 stifles demonstrated complete medial patellar luxation resolution. The study demonstrates that TPLO-TTT is an effective surgical approach for managing concurrent cranial cruciate ligament rupture and high-grade medial patellar luxation, with a relatively low major complication rate. However, owners should be counseled regarding the potential necessity for implant removal.
