This case report describes a 10-year-old male castrated Somali cat presenting with neurologic signs, severe systemic hypertension, and hypokalemia. Diagnostic imaging identified a left adrenal mass with elevated serum aldosterone concentration, consistent with primary hyperaldosteronism. Computed tomography confirmed a functional adenocarcinoma of the adrenal gland, with concurrent chronic airway and parenchymal disease. Transthoracic echocardiography revealed significant cardiac pathology including biatrial enlargement, abdominal effusion, and pericardial effusion, indicating right-sided congestive heart failure. The cat underwent adrenalectomy following initial congestive heart failure management. Remarkably, cardiac structure and function normalized completely within 6 months postoperatively. This case demonstrates that congestive heart failure secondary to primary hyperaldosteronism can be reversible with appropriate surgical intervention targeting the underlying etiology. The reversibility of cardiac changes following adrenalectomy suggests that aldosterone-mediated pathophysiology, rather than irreversible structural remodeling, was the primary mechanism of cardiac dysfunction in this patient.
