This study investigates ferroptosis as a pathogenic mechanism in feline chronic kidney disease (CKD) by evaluating urinary glutathione peroxidase 4 (GPX4), the primary enzymatic inhibitor of lipid peroxidation. Eighty-two cats were evaluated: 15 healthy controls, 61 with CKD, and 6 with acute-on-chronic kidney disease. Urine GPX4 levels and the urine-GPX4-to-creatinine ratio (UGCR) were measured via ELISA. Late-stage CKD cats demonstrated significantly elevated urine GPX4 and UGCR compared to controls and early-stage CKD cats. Within CKD subgroups, UGCR correlated significantly with proteinuria, hypertension, anemia, and iron supplementation. Serum creatinine and WBC counts were independent variables associated with UGCR. Importantly, elevated UGCR predicted CKD progression (HR 1.75, 95% CI 1.20-2.54, p=0.003). The findings suggest urine GPX4 reflects ferroptotic activity in renal tissue and may serve as a novel non-invasive biomarker for assessing CKD severity and predicting disease progression in cats.
