This study investigates calciprotein particles (CPP), nanoparticles implicated in chronic kidney disease-mineral and bone disorder (CKD-MBD) pathogenesis, in cats with naturally occurring azotemic CKD. Fifty-two cats stabilized on phosphate-restricted diets were evaluated for total CPP, low-density CPP, and high-density CPP using fluorescent bisphosphonate detection after gel filtration. Fibroblast growth factor-23 and parathyroid hormone were significantly associated with preprandial total CPP concentrations in phosphate-restricted fed cats, while phosphate correlated with postprandial CPP concentrations before dietary restriction. Changes in total CPP differed significantly between cats with increasing versus decreasing ionized calcium trajectories following diet stabilization. This research demonstrates CPP measurement feasibility in cats and suggests CPP assessment enhances CKD-MBD evaluation, particularly regarding soft tissue mineralization risk. Ionized calcium trajectory after phosphate-restricted diet implementation may influence CPP concentrations, providing new insights into mineral metabolism dysregulation in feline CKD.
