This case report describes the novel use of intramuscular mesenchymal stromal cell (MSC) therapy in a 7-year-old cat with severe asthma. The patient presented with chronic clinical signs including coughing, apathy, vomiting, and dependence on corticosteroids and bronchodilators. The cat received three IM injections of 1 × 10⁶ MSCs per kg body weight at 30-day intervals. Following treatment, the cat experienced sustained clinical improvement with successful discontinuation of corticosteroids and bronchodilators without recurrence of asthma attacks during 2 years of follow-up. Additional benefits included resolution of frequent vomiting and improved appetite with 0.6 kg weight gain. Quality-of-life assessment demonstrated significant improvement in vitality and comfort (P < .05). No adverse effects were documented. This case represents the first reported use of intramuscular MSC delivery for asthma in any species, offering a promising alternative therapeutic strategy to minimize long-term corticosteroid dependence in feline asthma patients while improving overall quality of life.
