This retrospective study evaluated the long-term safety and efficacy of oral bezafibrate in 55 client-owned dogs with hypertriglyceridemia. Dogs were treated with bezafibrate at a median dose of 5.5 mg/kg once daily and stratified into three groups: primary hypertriglyceridemia (n=group 1), secondary hypertriglyceridemia without concurrent treatment changes (n=group 2a), and secondary hypertriglyceridemia with concurrent treatment changes (n=group 2b). Serum triglyceride concentrations, creatine kinase, and alanine aminotransferase activities were monitored at baseline and follow-up intervals up to 18+ months. All groups demonstrated significant triglyceride reduction (P<0.01), with a median decrease of 85% across the study period. No significant differences in final triglyceride concentrations were observed between groups (P=0.13). Adequate treatment response (≥50% triglyceride reduction) was achieved in the majority of dogs. Adverse effects possibly attributable to bezafibrate were noted in only 4/55 dogs, primarily gastrointestinal or hepatic in nature. The findings support long-term bezafibrate use as a safe and effective treatment option for managing both primary and secondary hypertriglyceridemia in dogs.
