This retrospective study evaluates nerve-sparing total prostatectomy outcomes in 22 dogs with early-stage prostatic tumours (T1-T3, N0M0) treated between 2018-2024. Patients underwent complete preoperative staging including contrast-enhanced CT and urethrocystoscopy. Surgical time averaged 72.5 minutes with minimal complications: one minor intraoperative complication and no major postoperative complications. Postoperative urinary continence improved significantly during follow-up, with 68.2% achieving full continence and 31.8% experiencing only mild persistent incontinence. No dogs developed severe permanent incontinence. Tumour progression occurred in 27.3% of cases at a median of 150 days post-surgery. Median follow-up and survival times were 570 and 900 days respectively.
