This retrospective study evaluated 100 dogs with proteinuric chronic kidney disease to determine whether urine protein electrophoresis (UPE) patterns predict disease progression and survival. Using sodium dodecyl sulfate-agarose gel electrophoresis (SDS-AGE), dogs were classified into three groups: glomerular (n=47), mixed (n=40), and tubular (n=13) profiles. Dogs with tubular patterns demonstrated significantly accelerated disease progression, reaching a 25% increase in serum creatinine within a median of 2 months compared to 24 months in glomerular cases. The creatinine slope over the first year was substantially steeper in tubular cases (56 vs. 7 µmol/L/month). Most notably, tubular patterns were independently associated with markedly reduced CKD-related survival (median 1 month vs. 55 months for glomerular cases). Mixed patterns also predicted worse outcomes than glomerular patterns (median 12 vs. 55 months). These findings suggest that SDS-AGE profiling provides prognostic information independent of other biochemical variables and should be incorporated into the initial evaluation of proteinuric CKD in dogs.
