This retrospective cohort study compared pancreatic lipase activity (LIPC) and pancreatic lipase immunoreactivity (PLI) in 21 cats with suspected pancreatitis, tracked over a median of 6 weeks with a median of 2 re-checks per cat. Both assays showed moderate and comparable correlations with clinical disease activity score (CDAS) during follow-up visits (lipase activity rₛ = 0.486; PLI rₛ = 0.432), but neither correlated significantly at initial evaluation. Median values for both lipase assays and CDAS decreased significantly from initial presentation to follow-up. Seventeen cats had concurrent chronic enteropathy, and immunomodulatory therapy was more frequently administered during follow-up visits. Discordant results between assays were rare (4% of visits), occurring only in clinically mildly affected cats. The findings suggest both lipase assays provide clinically equivalent diagnostic information in feline pancreatitis, though the moderate correlation with clinical status only during follow-up may reflect the limited and nonspecific clinical signs characteristic of feline pancreatitis.
