This case report describes a one-year-old Kooikerhondje dog presenting with a heart murmur and echocardiographic findings of left heart enlargement and mosaic flow in the main pulmonary artery. Advanced imaging using ECG-gated computed tomography angiography revealed a unique congenital anomaly: two concurrent arteriovenous shunts converging into a common trunk draining into the left pulmonary artery. The first shunt originated from the right costocervical artery, coursing tortuously through the thorax before entering the left pulmonary artery. The second shunt arose from the left coronary artery with the same destination. Both anomalous vessels were successfully ligated via thoracotomy at their entry points into the left pulmonary artery. Post-operative follow-up imaging at 234 days confirmed complete resolution of both shunts with no recurrence. This case represents the first documented concurrent right costocervical artery-left pulmonary artery and left coronary artery-left pulmonary artery shunt in canine medicine, highlighting the diagnostic capabilities of ECG-gated CTA in identifying complex vascular anomalies and demonstrating successful surgical management.
