This prospective validation study evaluated the accuracy of two Doppler and two oscillometric non-invasive blood pressure (NIBP) devices against invasive arterial blood pressure (IBP) measurements in eight conscious Beagle dogs. Dorsal pedal arterial catheters were placed under anesthesia and measurements were collected once dogs were conscious, at both tail and forelimb cuff locations. Accuracy was assessed using American College of Veterinary Internal Medicine (ACVIM) validation criteria, which require bias ≤10 mmHg, standard deviation ≤15 mmHg, ≥50% of measurements within 10 mmHg of IBP, and adequate correlation coefficients. No device passed ACVIM validation criteria at either cuff location. Tail placement consistently outperformed forelimb placement across all devices. Doppler devices demonstrated superior accuracy for systolic blood pressure, while oscillometric devices performed better for mean arterial pressure (MAP). The best-performing device was a multiparameter oscillometric monitor measuring MAP at the tail, achieving a bias of 1.75 mmHg (SD 8.98), with 82% and 96% of values within 10 and 20 mmHg of IBP, respectively, and limits of agreement of -15 to +19 mmHg. However, the correlation coefficient threshold was not met by any device. These findings highlight significant limitations of currently available NIBP devices in conscious dogs and underscore the need for improved validation standards and device development. Clinicians are urged to interpret NIBP measurements cautiously and consider the clinical context when making treatment decisions based on non-invasive readings.
