This retrospective study evaluated whether preoperative CT-derived renal parenchymal volume could predict long-term postoperative renal function in 24 cats with hydronephrosis compared to 13 control cats with normal kidneys. Renal parenchymal volumes were normalized to body weight and analyzed against postoperative serum creatinine concentrations and chronic kidney disease (CKD) staging. Non-CKD hydronephrotic cats demonstrated significantly larger contralateral kidney parenchymal volumes than controls and CKD cats. In non-CKD cats, contralateral renal parenchymal volume showed strong negative correlation with postoperative serum creatinine (r = -0.79), while total renal parenchymal volume demonstrated moderate correlation (r = -0.46) in all hydronephrotic cats. Contralateral renal parenchymal volume/body weight was significantly greater in International Renal Interest Society stages I-II versus III-IV. The findings suggest that preoperative CT-based quantification of contralateral renal parenchymal volume is a useful predictor of postoperative renal function and CKD staging in cats with hydronephrosis undergoing surgery.
