This study evaluated cycle length variation as a diagnostic tool for differentiating three common equine arrhythmias: sinus arrhythmia (SA), atrial premature complexes (APCs), and ventricular premature complexes (VPCs) in resting horses. Thirty ECGs (10 per arrhythmia type) were analyzed by measuring sequences of RR and PP intervals, with percent deviations calculated both between sequential intervals (%ΔSEQRR, %ΔSEQPP) and compared to averaged baseline intervals (%ΔAVGRR, %ΔAVGPP). Results demonstrated that cycle length variation successfully differentiated SA from APCs and VPCs based on degree of prematurity and return cycle length using both interval types. Notably, %ΔAVGRR, %ΔSEQPP, and %ΔAVGPP outperformed %ΔSEQRR in distinguishing SA from APCs regarding prematurity. The %ΔSEQPP method was particularly valuable, differentiating APCs from both SA and VPCs in both prematurity and return cycle length. Methods using averaged baselines (%ΔAVGRR and %ΔAVGPP) eliminated the influence of prematurity on return cycle length variability. Post-arrhythmia sinus cycle variability (PP7–10) was observed following VPCs and SA. The incorporation of PP interval analysis provided enhanced discriminatory capacity, especially between APCs and SA, which has historically been a challenging differentiation. The authors acknowledge classification bias as a study limitation. Overall, cycle length variation analysis represents a practical adjunct tool for resting ECG interpretation in horses, improving arrhythmia classification accuracy in equine cardiology.
