This case report documents the first in vivo retinal changes in uveodermatologic syndrome (UDS) using optical coherence tomography (OCT) imaging in a 7-year-old mixed-breed dog. The patient presented with chronic panuveitis, bilateral bullous retinal detachments, and newly diagnosed vitiligo confirmed by dermatopathologic examination. Comprehensive ophthalmic evaluation combined with RetCam fundus photography and OCT b-scans revealed characteristic retinal pathology including tapetal hyperreflectivity, hyperpigmented scarring, and depigmented regions. OCT imaging demonstrated normal retinal layering in unaffected areas with marked structural alterations in affected regions, including partial-to-complete loss of retinal thickness and organization. Treatment with GLS-1027, a novel anti-inflammatory agent, resulted in clinical stabilization with no recurrence of signs over 12 months of follow-up. This case highlights OCT's diagnostic and monitoring potential in UDS, a rare systemic condition affecting multiple organ systems. The imaging findings correlate well with clinical manifestations and disease progression, establishing OCT as a valuable tool for detailed characterization of retinal involvement in UDS and supporting its use in longitudinal disease surveillance.
