Clinical Features and Surgical Outcomes of Cats With Presumed Primary Lens Instability: A Retrospective Study of 34 Cases (2018-2022).

This retrospective study analyzed 34 cats (68 eyes) with presumed primary lens instability (PLI) presenting between 2018-2022. The cohort was predominantly young (median 3 years) and male (82%). At presentation, 68% of eyes had anterior lens luxation (ALL) and 32% had subluxated lens (SLL). Notably, 73% of SLL eyes progressed to ALL within a median of 15 days. Elevated intraocular pressure (>25 mmHg) was documented in 38% of eyes overall, with higher prevalence in SLL (62%) compared to ALL (25%). Forty-four eyes underwent surgical intervention via intracapsular lens extraction or phacoemulsification. Postoperative outcomes in 42 eyes with adequate follow-up (median 284 days) were favorable for comfort (95%) and vision retention (85%). However, 70% developed postoperative glaucoma within 68±51 days, persisting in 35% at final examination. Retinal complications occurred in 14% of surgical cases. Notably, nonsurgical eyes also demonstrated high IOP elevation risk. The study concludes that PLI in cats presents with bilateral disease, variable clinical signs, and significant glaucoma risk despite surgical intervention.
