This case report describes the successful conservative management of bilateral palpebral squamous cell carcinoma (SCC) in a domestic shorthair cat using photodynamic therapy (PDT) alone and in combination with electrochemotherapy (ECT). The cat presented with histologically confirmed SCC affecting both eyelid margins. The left eyelid was treated with a single PDT session using topical methyl ester derivative of 5-aminolevulinic acid (160 mg/g) as a photosensitizer activated by red LED light, achieving complete clinical resolution. The right eyelid received two PDT sessions, which produced significant but incomplete regression of neoplastic lesions; a residual medial canthal lesion subsequently underwent adjuvant ECT with intravenous bleomycin, resulting in complete clinical resolution with only mild eyelid deformity. PDT demonstrated particular efficacy for superficial lesions, while the addition of ECT addressed deeper or more resistant disease. This sequential multimodal approach successfully preserved eyelid function and aesthetic integrity, avoiding the need for extensive surgical excision that could compromise ocular protection and vision. Palpebral SCC is the most common feline ocular tumor, frequently affecting depigmented or lightly pigmented eyelid margins, and surgical management can result in significant functional and cosmetic deficits. This case supports PDT and ECT as viable, minimally invasive alternatives for feline palpebral SCC, offering tumor control with preservation of eyelid architecture. Further prospective studies with larger patient cohorts are needed to establish standardized protocols and long-term outcomes.
