This retrospective study evaluated surgical outcomes in cats with glaucoma treated with endoscopic cyclophotocoagulation (ECP) combined with lensectomy between 2020 and 2025. Twenty-two eyes from 15 cats were included, representing a variety of glaucoma etiologies including primary open-angle glaucoma, feline aqueous humor misdirection syndrome, chronic uveitis, lens subluxation, anterior segment dysgenesis, and toxic anterior segment syndrome following phacoemulsification. All patients underwent lensectomy via phacoemulsification or intracapsular lens extraction prior to ECP. Successful IOP control (defined as ≤30 mmHg) was achieved in 86% of cases at three months and 94% at one year. Vision was present in 77% of eyes at three months and 71% at one year. Notably, among eyes that were visual preoperatively, vision was retained in 93% at three months and 100% at one year, suggesting the procedure is particularly effective at preserving existing vision. No significant effect of signalment on IOP control or vision retention was identified at any evaluated time point. The most frequently reported postoperative complication was corneal ulceration, occurring in 36% of cases, which warrants careful postoperative monitoring.
